The final chapter details lessons to be learned from the crash, including what the authors call a "potent formula for disaster"; information asymmetry; absence of slack; cognitive biases; overreaching; shock occurs; and, an inadequacy of response.

Jay Snyder is a voice actor, voice director, and script adapter who studied acting at the Julliard School in New York City. He is best known as the voice of Yugi Muto from the Japanese manga television series, Yu-Gi-Oh!
